For most of human history, the response to compulsive gambling was punitive — exile, imprisonment, public humiliation. This module traces the development of evidence-based treatment from those punitive origins through the current gold standard of cognitive-behavioral therapy, motivational interviewing, and adjunctive pharmacotherapy. It presents the evidence clearly, acknowledges what remains uncertain, and frames recovery as achievable without overstating the evidence base.
For most of human history, the response to compulsive gambling was punitive. Medieval and early modern societies — understanding it as moral failing or criminal behavior — responded with exile, imprisonment, public humiliation, and in documented cases, corporal punishment. These interventions proved entirely ineffective; individuals continued to gamble despite severe consequences, a pattern that itself constitutes evidence of a condition driven by mechanisms more complex than simple moral weakness or lack of will.
The twentieth century marked a turning point. Gamblers Anonymous provided the first scalable, peer-based framework for recovery — and its Twelve Step model remains widely used today. GA’s value lies primarily in its accessibility, its community structure, and its capacity to sustain engagement over the long term for motivated individuals. Its limitations are equally important to acknowledge: GA advocates complete abstinence and relies on spiritual principles that are not universally acceptable; dropout rates are high; and long-term abstinence rates for GA participation alone appear modest. For many individuals, GA functions best as an adjunct to professional treatment rather than a standalone intervention.
The current gold standard for evidence-based treatment is cognitive-behavioral therapy (CBT). CBT for gambling disorder targets the cognitive distortions that sustain gambling behavior — particularly the gambler’s fallacy, illusions of control, and selective recall of wins relative to losses. Standard protocols include psychoeducation about probability and randomness, structured cognitive restructuring, problem-solving and coping skills training, stimulus control strategies, and relapse prevention planning. Meta-analyses consistently demonstrate significant reductions in gambling frequency, financial losses, and urge intensity, with effects generally maintained at six- to twelve-month follow-up.
Motivational interviewing (MI) addresses the ambivalence that characterizes many individuals with gambling disorder. MI can be delivered as a brief standalone intervention or integrated as an opening phase of CBT. Its primary function is to strengthen intrinsic motivation and resolve ambivalence before skills-based work begins, improving engagement and reducing early dropout.
Pharmacological treatment has an emerging but still limited evidence base. Opioid antagonists — naltrexone in particular, with nalmefene also studied — have demonstrated efficacy in multiple randomized controlled trials, reducing gambling urges and behavior. Naltrexone is currently the most supported pharmacological option and is reasonable to consider as an adjunct to psychotherapy in moderate-to-severe presentations. Pharmacotherapy for gambling disorder should generally be combined with psychological treatment, not substituted for it.
CBT effect sizes for gambling disorder in meta-analyses (e.g., Rash & Petry, 2014) are moderate to large (d ≈ 0.6–1.0 for gambling frequency and urge measures). Brief MI interventions (one to four sessions) have demonstrated reductions in gambling frequency and problem severity in RCTs, suggesting a meaningful role in primary care and emergency department settings.
Naltrexone dosing in gambling disorder trials has ranged from 50–150 mg/day. Higher doses (100–150 mg) have shown larger effect sizes in some trials, though tolerability declines at higher doses. LFT monitoring is indicated at standard addiction psychiatry thresholds. Nalmefene (not FDA-approved in the US but used in Europe) has demonstrated efficacy in gambling disorder RCTs.
Comorbidity is the rule, not the exception: rates of co-occurring mood disorders, anxiety disorders, substance use disorders, and ADHD are substantially elevated in clinical gambling disorder populations. The treatment-seeking gap remains large: fewer than 10% of individuals with gambling disorder access specialty treatment.
